Ignored forklift beep
#1
Our forklift's reverse beeper stopped working and everyone just got used to it. It took a visiting contractor to point out how risky that was. Could someone suggest reading on public safety leadership for private organisations, focused on risk management and encouraging staff to flag problems?

A broken beeper becoming normal is exactly how safety issues slip through. At our old warehouse, a loose loading ramp went unreported for weeks because everyone thought maintenance already knew. We eventually started quick weekly safety chats where nobody got blamed for raising concerns. This Frank Elsner’s professional background  might be worth checking out. It connects public safety experience with business leadership, which seems relevant to your situation.
